Nile’s Karl Sanders Recovering From Spinal Fusion Surgery, Expects to Tour Europe Nov. 29

Nile’s Karl Sanders Recovering From Spinal Fusion Surgery, Expects to Tour Europe Nov. 29

Nile guitarist and vocalist Karl Sanders is recovering from multilevel spinal fusion surgery in his neck, and he says he expects to be back on the road when the band’s Underworld Europe 2026 Tour opens Nov. 29 in Athens, Greece. Karl Sanders shared the update on social media on Thursday, Oct. 22, writing that the operation took place Oct. 1, according to Blabbermouth.

Why Karl Sanders Needed Spinal Fusion Surgery

In his post, Sanders described problems that had been getting progressively worse. He listed disc degeneration, “decades of headbanging,” a martial arts injury 11 years ago and “a long stretch of stress” as factors that had taken a toll on his health.

He wrote that he had been dealing with pain from pressure on his spinal cord and nerves, but that constant numbness in his right hand this year convinced him he could not put it off. “Scans showed severe narrowing around my spinal cord, along with signs of injury to the cord itself,” he wrote. “My doctors made it clear that surgery was necessary. Without it, I was facing further permanent damage, including a very real risk of paralysis.”

Sanders explained that during the procedure, the damaged discs and bone pressing on his spinal cord were removed. Donor bone was placed between multiple vertebrae, which were secured with a metal plate and screws. “So there’s officially a little more Metal in me now,” he added.

Recovery and the European Tour

“The surgery went well, and I’m putting in the work on recovery,” Sanders wrote. He said his neck and shoulder pain is easing and his throat is healing. He is walking two to three miles a day, doing occupational therapy for his right hand three times a week, following his doctor’s instructions “and playing LOTS of guitar.”

“Eight weeks to the day after surgery, I’ll be back on the road for our European tour,” he wrote. “I’m looking forward to getting back out there.”

He also noted the financial side of the situation: “Making music is what I love, but it’s also how I make my living, and metal doesn’t come with paid medical leave.”

One thing will change onstage. “I won’t be doing any more onstage headbanging, but that’s OK,” Sanders wrote. “I have done more than my share of headbanging during my career, and I am thankful that I can still actively tour and Make Metal for the fans.”

New Nile Album in Progress

The health news comes while Nile is writing the follow-up to its 10th album, “The Underworld Awaits Us All,” which came out in August 2024 via Napalm Records. SDMETAL covered the album’s release week, when the band shared the single “Under the Curse of the One God”.

Last month, Sanders told Rev. Tom Brice of Sportzwire Radio, as quoted by Blabbermouth, that the band was “working on song number six right now.” He praised the current lineup, saying, “this new lineup of guys we got is just on fire,” and called the material so far “some really, really killer shit.”

Asked why a new record is taking time, he said that after 10 records he thinks “it’s way smarter to get it right,” adding: “Take your time. Get it right. The fans will appreciate it.” No title or release date for the new album has been announced.

The lineup has shifted since the last record. Nile recruited bassist and vocalist Adam Roethlisberger in September 2025 after Dan Vadim Von announced his departure on Aug. 20, 2025. Roethlisberger made his live debut with the band on Sept. 12, 2025, in Raleigh, North Carolina, at the opening show of a U.S. tour with Cryptopsy.

SDMETAL’s 2013 Interview With Karl Sanders

SDMETAL spoke with Sanders in 2013, during the band’s U.S. tour behind its album “At the Gate of Sethu,” as Nile marked 20 years together. In the video interview, Sanders talks about the demands of playing two full sets a night, the bands’ decision to bring local openers on that tour and his time aboard the 70,000 Tons of Metal cruise. The full interview, titled “SDMETAL chats with Karl Sanders from Death Metal band Nile,” is on SDMETAL’s YouTube channel.

Nile’s San Diego Date Still Listed

Closer to home, Nile is booked to co-headline with Swiss band Coroner, with support from Syn Absence, on a North American tour that runs from March 2, 2027, in Greenville, South Carolina, to April 11, 2027, in Leesburg, Virginia. The tour includes a stop at House of Blues San Diego on Sunday, March 21, 2027. Details on the Underworld Dissonance Tour stop, including the all-ages policy, are on SDMETAL’s event listing.

Karl Sanders’ update did not mention the San Diego show or the North American tour. As of Oct. 22, the House of Blues date was still listed on Do San Diego, with doors at 6 p.m. and the show at 6:30 p.m. The listing says the event is all ages, with anyone under 18 required to be accompanied by a parent, guardian or authorized adult 25 or older, and notes that all times and supporting acts are subject to change.

House of Blues San Diego is at 1055 Fifth Ave. in the Gaslamp Quarter.
